This policy describes how Strive PT & Performance (“Strive,” “we,” or “us”) handles information collected through striveptandperformance.com(the “Site”).
Information we collect
Booking data. When you schedule an appointment through our booking widget, you interact directly with Square Appointments. Square collects your name, contact details, and payment information under Square’s Privacy Policy. Strive receives only the appointment confirmation details Square shares with practitioners.
Usage data. When you browse the Site, our servers and analytics may log standard technical data such as your IP address, browser type, pages visited, and referring URL. This data is used solely to understand how the Site is used and to maintain security. We do not use third-party advertising trackers.
Resource interactions. When you open a resource on the Site, we record an anonymous click count (no personal data) to understand which content is most useful. This data is stored in our content management system and is not linked to any individual.
How we use your information
We use information collected through this Site only to operate the Site, schedule appointments, and improve the patient experience. We do not sell, rent, or share your information with third parties for marketing purposes.
Third-party services
The Site uses the following third-party services, each governed by its own privacy policy:
- Square Appointments — online scheduling and payment processing
- Sanity — content management (resources, photos)
- Vercel — website hosting and edge delivery
Health information & HIPAA
This Site does not collect protected health information (PHI). Health information you share during the course of your care is governed by Strive’s HIPAA Notice of Privacy Practices, which is provided at your first appointment and available on request.
Cookies
The Site uses only functional session storage (e.g., to remember that you dismissed a popup). No advertising or cross-site tracking cookies are set.
Data retention
Server logs are retained for up to 90 days for security purposes and then deleted. Booking records are retained as required by Massachusetts healthcare regulations.
Your rights
You may request access to, correction of, or deletion of any personal information we hold about you by contacting us at the address below. If you are a Massachusetts resident, you have additional rights under M.G.L. c. 93H.
Changes to this policy
We may update this policy from time to time. The effective date at the top of this page will reflect the most recent revision.
